Moisture touched a hot bulb that's what it looks like to me

Also touch to bulb with out a clean rag or gloves. If you touch the bulb with no glove. The oil on your hands will get on the bulb and cause it to blow when the bulb starts to get heat.

I've seen it happen from when the bulb cools to fast causes confiscation so the next fire up it pops it

Yea from the seal at the base of the bulb were the glass meets the metal I know it sounds crazy but I've seen two bulbs do it
Better the inside glass 'pouch' than the external bulb itself. I have run all kinds of MH bulbs since about 1987 and have had this happen a few times. Who knows why - condensation maybe, slight flaw in the glass, ballast pushing I too much ..... I also had the glass shield on my HQI fixture blow one time, safety glass chunks all over the sand bed, ugh.
