Hi guys

I would really appreciate some help. My Male Lyretail Anthias has a gaping hole under its gill flap and breathing heavily. I just noticed it today when she was breathing hard. She has been eating very well. I would really appreciate if someone can tell me what it might be.
My tank parameters are below:

Salinity:1.026
Ca: 410
Alk: 7.0
Mg:1210
PO4:0.04
NO3: 0.02
I have a 75 gallon mixed reef with T5s.

I have another female lyretail anthias. There are no agressive fish in my tank...just regular clowns and small wrasses. I have had the male lyretail for about 2 years now.

I have the following medicines
Melafix
Pimafix
Formalin
Mardel Coppersafe
Prazipro

Thanks for the help.
