What color should the skimmate be? My skimmer has been running and producing foam, but the collection cup comes out with clear water.

dark coffee brown is what you want to ultimately achieve, Otherwise just try to get it as dark as you can.

Should the bubbles just barely be breaking the top of the reaction chamber?
