Can Anyone Identify what is wrong with my clown fish?

I have had this clown for about a week and a half and at first it was acting weird in my tank going up for air.. Tested all of my water parameters and everything was perfect but still performed a 20% water change to ensure the parameters were still perfect. About 2 days after the water change I noticed something started growing on his face. I have attached 2 video links in hopes that someone can identify and possibly advice me on how to help this little guy get better. He has not eaten in about 4 days so I'm starting to get worried. His diet previously consisted of spectrum pellets and mysis shrimp.

Maybe not enough room for him?

How old is the tank?

He's looks like he's breathing really fast, maybe gill flukes? I myself, might try a FW dip and then QT him.
 
I was in the process of setting up a QT Tank and was going to transfer him to it tomorrow but when I went to check on him he had passed... Checked the Salinity and it was 1.24 now.. Tank was already 2 months old.. Mandarin in there is doing excellent.. He eats mysis shrimp twice a day and hunts all day long for copepods.. I believe the stock aqueon pump does 100 gallons per hour and my pH was perfect so I couldn't see why he wasn't getting enough oxygen.. Considering my amonia non existent and my nitrites were the same I don't know what triggered what ever was on his gills.. When i checked him he had a whole under one of his gills by his mouth.. Guess i'll never know :(
 
I guess its a little late. I can't tell much from the video, other than an obvious problem around the face. Was the skin in this area peeling or sloughing off? how long have you had the clown? When was the last fish introduced? Parasites love the gills, but brooklynella is a parasite that also causes skin eruptions. I'd Google "marine brooklynella" and see if it makes sense. I sure wouldn't add any fish for a while. Congrats on having one of those rare mandarins that can survive on frozen food.
 
I'm still a little confused about the salinty. 1.24 and 1.024 are a big difference. I could run to my hydrometer and check it, but I'm pretty sure there is a "0" in the numbers. However, it might hurt everything, not just one fish.

He doesn't necessarily need a trigger to get sick (IMO) but the good stable water parameters do help with any chance of beating the disease. Since he was "new" and not QT'd I wouldn't put anymore fish in and just wait and see if the other fish gets sick.

Sorry for your fishie loss, I hate when that happens :\
