Clownfish Gill Issue

I have a semi picass clown fish that I noticed a strange gill extension. I was reviewing some diseases of fish but none seemed to apply to this issue. I did notice the fish having trouble swimming and placed them into a shallow bowl for inspection. There was no noticeable slime, mucus or other tell tale signs. I did how ever see one red spot on the body and the picture of the gills below that after checking other photos appears normal for clown fish. If you have seen this I would appreciate your input.

As far as tank mates there is a a grade a picasso, copperband butterfly, potters wrasse in a 20G holding tank plumbed to the main system. All tank parameters are stable and there are no signs with the other fish in the main display.

Currently I have removed the clown and placed them into a small critter cage floating in my sump pending a possible treatment.

Picture cropped and highlighted for the gills. This is not my fish but a photos offline that showed exactly what I saw upon inspection.
