Clownfish issues

I'm scratching my head as to what could be causing problems with my clowns...I lost a B&W a few weeks ago, and now all of the sudden the other clown is acting listless and appearing to be breathing heavily. A little background:

- Tank is a 30 gallon that has been running with pre-cycled rock since beginning of Jan.
- Other inhabitants are another B&W clown, orchid dotty back, hermit crabs, some zoas, hammers, and ricordea.
- I got the original B&W and the regular at the same time (about a month ago). I didn't QT them, and all seemed fine, except slowly the B&W got less interested in eating and then one day just kind of losing bouyancy and died pretty quickly.
- When I got the replacement clown, I set up a 10 gal QT tank and QT'd both of the clowns, because the regular one appeared to have a little discolored spot of its side (kind of looked like some type of infection).
- Clowns did great in QT (5 days), and I treated with copper (Cupramine) and maracyn.
- I put them both back in the display yesterday morning and all seemed well last night and even earlier this morning. Both eating well and swimming around. All of the sudden the regular clown just looks awful.
- Tank parameters for main items are: Ammonia - 0, Nitrite - 0, Nitrate - 0, pH - 8.1-8.2.

I don't have a phosphate test kit, but orchid dotty back is and has been doing great, and all corals have been looking happy. I'm doing weekly water changes of about 15%.

I'm kind of at a loss, I'm going to put the clown back in QT to see if anything changes...is there something I am missing or that is in the tank that could be causing this? All I am running is carbon and filter floss (changed out every couple of days). I have not seen any signs of aggression from any of the fish towards each other.


Grateful for any thoughts or suggestions.
 
Generally people quarantine for more than just 5 days. The clown could have been carrying something that just took longer than 5 days to take affect.
 
I've had this clown for about a month...its been fine up until late this morning, and he went downhill really quickly...
