Mushroom.
Not sure about what kind though.
Its not a discosoma.
But could be rodectus, richordea or yuma.
I will bet on rhicordae!

Do you know what placement, flow, and light they need?

They are low to moderate light. They prefer lower part of the tank, including on a rock on sand bed.
They dont like direct fliw and would detach themselves if they come under direct/harsh flow. Like, indirect low flow.
Generally slow growers but split by budding if they are happy.
They definitely appreciate feeding. I feed mine, a squirt or two of reef chilli.
