Cynarina Button Coral dying?

I have had this coral for over 6 months and all of a sudden it has gone from fully extended almost the size of a salad plate to now shriveled up to its skeleton (about the size of a golf ball)

I used to feed it chunks of scallop a long time ago, but lately it has just been getting light.

Do you think it's dying? Anyway to reverse it?

all parameters are stable, but I've seen a jump in my nitrates lately to over 25. Trying to do more water changes to bring that down
