Dead Foxface... Diagnosis??

So I lost my onespot foxface over the weekend... Seemed fine as recently as thursday. Friday he seemed to be hanging out in the corner of the tank where he 'sleeps', but came around when I was near the tank and went after some pellets. Sat he started looking rough, not swimming, just parked under a rock. Never noticed anything on his body, eyes looked fine. Today he was just floating around, would swim a bit and I noticed his tail looked a little discolored and he had a spot on his torso that almost looked like a sting of some sort. A dot with some discoloration around it. I got him moved into my quarantine, but it was too late and he kicked the bucket this evening. I did get some pics and was hoping someone could id what the heck it was. After he died he discolored rapidly around his stomach area. Tank is a 120 and he was the largest fish. Only other inhabitants are a Kole tang, 2 clowns, 6 line and a cleaner shrimp. I've never seen the kole with his 'knives' out or anything aggresive at him. Everyone seemed good. I'm attemping to attach 3 pics. One in QT when I got him, one lying on the sand in the display where you can see the white dot low on his body and the 3rd is the autopsy photo where you can see his stomach changing colors... Everyone else in the tank seems fine, but i'm slightly concerned.

So I ended up also losing my Kole tang suddenly along with the foxface. I posted in general reef and someone noticed spots on the gills of the foxface and suggested a bacterial infection. Wondering if anyone here could confirm or provide some clues. The kole tang showed no signs of trouble. No outside marks... He seemed a little shy for the last 24 hrs, but he's always been a little skittish. He took food last night and this morning, not a lot, but he definitely ate, and when I got home from work he was dead. And I just saw the sticky, so i filled in what was appropriate below.

1. How old is this aquarium? - 3 years (approx)

2. If less than six months old, what is ammonia level? (n/a)

3. What is SG of this aquarium? How measured? 1.026 - refractometer

4. When was the last fish added to this aquarium? - 3 months ago

5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How? Fish was in my qt for 3 weeks, at LFS for 2. Was not treated with anything. Look and ate fine for the last 2 months.

6. If you are using a copper based medication, which one? How often do you measure level? When? n/a

7. If you are using hyposalinity, how did you calibrate your refractometer? n/a

8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color? Described above

9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?Described above

10. Is the fish eating? What?Described above
 
Pictures are insufficient for diagnosis. Time line is a bit long for a parasite which should show up in the 3-4 weeks from arrival from the LFS. I would extend the QT time, however, for subsequent purchases.
