Fish Dead..HELP

I have a 40g thats been up for over 3 months. I recently got a sixline wrasse 1 week ago. The tank already had 1 goby and 2 clowns. From the beginning he ate, and was active and healthy, although he had some faint white dots, and got more over time but were very faint. I found him dead last night in the rocks and I don't know why. Also my goby has 1 eye that has turned cloudy in the middle. Please help. I keep LPS and some SPS and all are doing well and growing.
My params that could affect the fish are

Ammonia 0
Nitrite 0
Nitrate 0
Ph 8.2
Phosphate .30 I know high, but i think my recent test is lying since my algae is getting better
 
Flukes, catch all fish and threat with prazi in a qt tank. Tank need to be fish free for some time (If I remember correctly ~5 weeks), but check on the exact duration.

If you are really desperate, you can use prazi in DT, but it will take a toll on the beneficial worm population.
 
It depends on what the "white dots" actually look like. A picture would help.

Generally ich are small white opaque balls on fins, eyes, and skin, more like grains of sand/salt. Flukes are generally larger, more like translucent light/dark colored spots on their skin, and not quite as obvious..
