Possibles include an onset of disease or the result of intestinal parasites of some sort, which leads to a reduced access to nutrition. Of course it could just be the foods and supplements are not right for this fish. There is always the possibility it is something in the water you are not testing for or you are unable to test for.

Aquarists check water for several components, but we know there are hundreds of things we don't test for that can affect the health of fishes.

I would take it one step at a time. Begin with a close review of its nutrition and use this reference as guidance:

Get and use the proper foods, supplements and element additives.

Next do a large water change. Be on the lookout for bad water sources (source water pollution) or anything that might be getting into the tank water (e.g., paint fumes, rusting equipment, copper sources, etc.). Repeat the water changes more frequently than you usually do and play investigator on the lookout for water contamination.

Be on the lookout for any signs of disease or illness.

Good luck!
