Frogspawn colony dying one head at a time ...

I've had this large (15 head) colony in my tank for a couple of months and about a week ago I started losing one head at a time. they would just slowly shrink up and then the head would just detach from the hard body.

I decided to aggressively frag teh colony in hopes of isolating whatever was happening and it looks like I've managed to save a few heads but the bulk are slowly dying off.

Anyone ever seen this before? It does not sem to be affecting any other coral in the tank.

In the first pic you can see the lower heads being affected as well as where a previous head already died off.

The next pic is of one of the frags which seems to be affected and the last pic is of one of teh frags that looks ok and one nearby which is not doing so well.

What are the parameters? Have you added any equipment? I addeda reactor with GFO and some of my corals start shiviling, so I removed it and started slowly. I wish I could be of better assistance, but I have never seen this issue. When you say the head falls off, does it melt off?
