Fromia Star -- Not Well

[Whys]

I've had a small red fromia star in my tank for the last few months. For the last few days it has barely moved and one of its legs appears to be coming loose. I have at least 40 lbs of healthy LR in the display, it has no competition, and little to nothing has changed in my tank since adding it. I do continue to do water changes at least once every 6 weeks.

Any thoughts on what might be wrong?

I had one and it wasted away like that. I think it starved to death.

Any sudden changes in the water params? Stars do not like anything that is not completely stable and are a complicated vert.
