Help Help my Hammer Head is Dying !

I've had this Hammer Head LPS for about 5 years. It has grown considerably, especially in the past year. A couple of weeks ago it started suffering, and in the process I've lost 4-5 heads and the death continuous!!! I took it out and dipped it in coral disinfectant from Kent a couple of days ago, it seemed to hold the damage but today I see it continues and I suspect if it continues I'll loose it in a couple of weeks.

Sorry LPS, and no new change from my levels and lighting that I've had for a while.

I would dip it again and then get it in a hospital tank so it can recover.
