Help! Sick/injured Coral Beauty

I have a Coral Beauty that has been having problems the last couple of days. He is laying on his side on the bottom and swimming around. He acts like his tail is broken. It never moves while he is swimming. The fish is still active and eating, just acts like it can't swim. I have added an air line to one of my mj1200s (for 02 saturation) and I have started lowering salinity. It is down to about 30ppt at this point. I figure those are the two natural things I can do to help him. I have also done a 20% water change and a couple more 5% or so changes. Any ideas what is wrong or anything else I can do? I have a 10g that was going to be a hospital tank, but I haven't added salt to it yet. Do I need to get it running ASAP? Would that stress him more?

Things I have changed lately:

Added RBTA. Already had a GBTA. Chemical warfare maybe? Nothing else seems affected. One clown is hosting it, the other in the rose. Firefish, Lawnmower Blenny, and Sleeper Goby are fine.

started dosing Espsom Salts. Over a period of about 4 weeks have added a 4 lb package, which is enough to raise it 400ppm, nothing extreme as Mag is not dangerous and I kept up weekly 20% water changes. Sulfate issues, maybe? Again, no one else cares, good PE on SPS, and all softies are fine.
