Help with Hammer Coral

Not sure if it's gone but trying to figure out what happened with my hammer coral and if there is anyway to save it.
First picture is from today and the second is a little less than a month ago. It wasn't doing great then but it's gotten even worse.

I've dipped it several times. After each dip I thought it was going to do better but no success.

Currently, I dose with Redsea NoPox.

I have had it for 1 1/2 years under LEDs (Vertex Illumina 260).

There was a point in time where I had an outbreak of GHA and my phosphates were elevated to about .16 but that was months ago.
 
This is the biggest problem with any of the wall type Euphyllia. I normally make it a point not to buy any because success rate is like 0 with any of them. Not sure what it is about the wall corals but this is all too common. It seems that mostly the wall types that have previously been fragged succumb to death faster than ones that weren't fragged.

Another thing to be watchful for is brown jelly disease. It can happen with most any corals but seems to be very bad with Euphyllias and Duncans. Here is a link about treating BJD with iodine. http://www.athiel.com/lib/bacterial.html

Success rate with any of the branching type Euphyllia is much better! All this said I purchased a wall hammer several months ago and it's doing great so far. You can see all of my corals here: http://www.reefcentral.com/forums/showthread.php?t=2540376 even a few wall types that I no longer have along with the one I mentioned earlier that is still doing well.

Care for any of the Euphyllia is the same no mater the type, they require low flow and low lighting. They will show you they are happy with extreme polyp extension.

I've got several new pictures to post this week and will start with some of them later today so keep checking my thread for the updated pictures. I've got new frogspawn, torches, a beautiful new Elegance and several acans that I just picked up this past Saturday. Also a lot of pictures of many of the acans I lost due to new lights burning them. All of this is in my thread.
