ID please

Got this little guy and not sure what it is. They said it is a plate coral but i looked up picture and it doesnt come up with that same guy. Also if you do know what it is its starting to bleach and i am not sure what is wrong with it. My Calcium is 500(little high i know). I have 130 watts on a 40 gallon tank so not sure what is wrong. i also put some phytopheast in because lfs told me to do that for him. Any one know what species and whats wrong with it?

looks like a monti cap.
the white edges look like new growth, not bleaching.

I dont think it is growth because there was a little bit of white when i got it and now the white has gotten bigger. I just checked my alk and it is 100ppm. Could that be the problem?

does any one know. In the pics you can see those tiny little holes are those bad? Also is it dieing or growing? The first photo you can see the little black holes.
