Is this ICH or Flukes or ?

After being gone for a week this is what I came home to. I did a water change and cleaned the sandbed prior to leaving. I removed the fish and placed him in QT. Did a quick set of parameters and found this
calcium 440
alkalinity 7dkh
magnesium 1200
nitrates and nitrites 0
phosphates undetectable.

Started raising the alkalinity and mag up over the past couple of days. Also started the fish on Prazipro in case this was Flukes. If this doesnt work I am going to do hyposalinity for the next 4-8 weeks.

One thing I dont understand is I havent added any fish for over a year and every fish was placed in QT for 4 weeks to insure they were healthy.

Any help would be greatly appreciated.

Looks like Ich to me. All fish will need to be QT and treated. Leave the DT fallow for the next 9-10 weeks to starve out the Ich in your DT.

Did you QT the fish, or the lfs? Four weeks is not long enough for a QT regimen to be as effective as it could be. I think 8 weeks is minimum.
 
Best advice at this point is to not delay in getting all fish into a hospital tank and treatment started ASAP!
Ich will be there one day then gone the next... just when you think your safe, it comes back with a vengeance and you'll be luck if it doesn't kill everyone in the tank.
This was my experience with it and my delay almost killed my fish, so don't let it fool you if it goes away or "gets better".
I chose to use Coppersafe as my Tx, I didn't feel my experience level was good enough at the time to safely keep a QT hypo. The copper worked quickly and was continued for 30 days, BUT I did lose a swallow tail angle to pop-eye 40 days into it. I have read that some angels don't tolerate copper well.
The DT stayed fish-less and the fish stayed in QT/HT for 96 days and so far no more ich.

That's what I don't understand. I haven't added anything new to the tank. Only thing I can think of was there was something in the sand bed when I cleaned it.
 
It's possible. I've seen crypto and amyloodinium rear their ugly heads in systems that have gone without any issue for over a year when one of two things happens: 1- the sand bed gets heavily disrupted, or 2- one of the fish becomes weakened and susceptible to re-infestation.
It may just be the image, but the hippo looks a little thin and the pec fin frayed. Could it have been in a weakened state? When you quarantine, is it just observational or do you medicate?
 
The fish has been in QT following the hyposalinity sticky. The fish still has spots everywhere. He is scratching at everything. Not sure where to go from here? Any advise on what this could be.

assuming your salinity is held constant at 1.008-1.009 with a calibrated refractometer, there's nothing really you can do right now except for waiting. eventually the trophonts will fall off and your fish should feel better.
 
I have checked the salinity with 2 different refractometers and both show 1.009.

Do your other fish show any symptoms of cryptocaryon? On this species I've seen bacterial infections that looked very similar. I am currently treating a friends powder blue tang which I could easily have confused with Ich if I hadn't confirmed with a skin scrape on a microscope. If the fish isn't getting better in 2 or 3 days after full hypo, consider antibiotics? Maybe hold off on removing all of your fish if it's not crypto?
