My anemone is sick

Hi i have an anemone its been in the tank for 4 weeks now and it imidiatley moved to the back. Some people told me to fliip the rock so i can see it because i couldnt. Now mu buble tip looks like this. Help please

how old is the tank bubble tips are very are very finicky with young Tanks. Was the tank properly cycled ?

The tank was properly cycled and it has over 1 year.
