Need help with Formalin use/dosage

I've decided to try Formalin for the issue my chevron tang is having...see this thread for details:

I have a 75g QT and the Formalin MS bottle states 1 teaspoon treats 90g. I added just under 1 teaspoon to the tank. The bottle also states to add this every other day...is this the proper protocol to use this?

A protocol is utilized for a specific disease...you don't know what you are treating so there is no way in knowing what protocol should be used. I personally would not use formalin unless I knew exactly what I was dealing with. It's extremely caustic and can be worse than the disease.
In case you have not dealt with formalin just a few tips..formalin will kill the biological filter so water changes are a must. Be very vigilant observing the fish as the effects of formalin can go south quickly so be ready to remove it. IME dips work better and are easier on the fish than baths. Keep us posted as to the fish's progress.

Decided to move him to a buddy's tank with good water conditions and we'll see what happens..I've had good luck moving other fish there and have made complete recoveries.

Maybe it's some kind of anchorworm. I would try a fw dip
