Ich has a certain lifecycle and it is possible to completely get rid of it. In many cases people don't wait long enough with hypo and never really allow the parasite to complete this lifecycle. Fish also can keep it at bay until they get stressed as stated above. i had a hippo tang that had ich and I did the hypo thing. When I went away I had an issue with my top off system and I came home to find the SG was at 1.33. Considering that the tang didn't get ich I have to believe it can be completely removed from a tank because the fish must have been very stressed. Your display must remain fallow for 6-8 weeks and your fish should remain in hypo for that period of time as well. You can decrease the time frame a bit by upping water temp, but patience has the best payoff.
