New Clowns in QT are showing signs of ICH/Velvet/Brook symptoms

I bought some fish from someone. They were all in the same system. I have them in a QT tank(divided up). I didn't medicate. I was only observing. After a week, they are starting to show white spots and one is also getting some grey on it's white bar. NOTE: I've never had any disease in anything in my systems.

Here is a video from last night - Black Snowflake

Occellaris - from this morning
Here is what I did so far last night.

5 min freshwater dip for all fish in the QT tank
50 minute formalin dip right after with Formalin MS - 20 drops per gallon
I added CP (active disease dose) to the tank - 80mg per gallon

I repeated the formalin dip this morning.

At first I thought this was ICH. But then I saw the grey spots and thought Velvet. Brook doesn't seem out of the question(early stage). I've seen Brook before and it's usually much much faster and the skin is usually much worse. However, I suppose I could have caught it early.

The black snowflake female has both eyes swollen(like a bubble).

Any thoughts? Is this multiple diseases? They came from the same system so that's why I am QTing them all in the same tank (separated to keep them from fighting).
