Newbie's first fish

Hello all. I have a nuvo 20 that I just set up. It's had a live rock set up for about 5 weeks. Two fish actually came with the rock. A seaweed blenny and a royal gramma. They've been doing great and eating fine until today. I never had any ammonia spikes, highest nitrite ever went was about .25, but the fish went through all this weeks ago and never showed any signs of distress.

0 ammonia, 0 nitrite, 40 nitrate (long story, but had a mantis that was hoarding food and actually hatched some eggs in the tank, so I've been doing water changes to bring it back down. But this isn't a new thing for the fish. Mantis is gone now.)

The only symptoms I've noticed were some flashing. No signs of ich or anything visible. I added a few beginner coral this past weekend after a coralrx dip and a tiny amount of chaeto in one of the back compartments after putting it in the fridge over night. All the coral, chaeto, inverts, etc seem to be doing fine. No signs of stress or anything.

But this morning my gramma looks on deaths doorstep. She was fine and eating yesterday. Now she can't swim against the current and is sitting on the ground. She's got some discoloration just above her face before her dorsal fin.

My blenny looks like he's been through a blender. His face looks ragged. Almost like skin is flaking off or something. Pic below for reference. He's also now swimming towards the top of the tank, which is unusual. And he doesn't look to be swimming as strongly as usual.

After a search this morning, I thought maybe we were looking at flukes, so I did a freshwater dip for 10 minutes, but nothing came off that I could see.

It just seems like they took a huge turn overnight and I can't figure it out. :(

Any other ideas? If neither of them make it, should I just leave the tank with no fish for a while just to kill off anything it could have been?

Honestly, it's hard to tell, but it may be brook or uronema. I would give them a Formalin bath for 50 minutes to start with.
