Polyp Problems

So recently I have been having a few problems with some of my polyps and so I took a few pictures and thought maybe someone here might have an idea.

This frag had one polyp randomly appear to deflate and turn clear, almost looking like a melted crayon. It tore when I was examining it for pests. The frag has been in my system for 18 months, and in that time has put on one polyp.

I found this guy on a rock and I had forgotten I had him, so Im not sure how long its been like this. No more than three weeks though. Its been in my system for about a year now.

And my SC Ultimate Chaos decided to stay closed up for a two weeks. The skirt pokes out of the opening most of the time, and the baby polyp stays open. I did see what might be zoa pox on the polyp so I thought Id take a picture. Its been in my system since August if I remember correctly.

I'm not sure what to look for on the first picture. It looks like some glue with algae on it to me. The polyps on the bottom left looks like they were ripped off or something. Perhaps there is a predator around. Check at night...

Second picture shows what I would believe to be a bacterial infection, probably after some type of predation too (or not). I would cut off the weird injured heads with a very sharp scissors and try a Lugol's solution dip. 8 - 10oz of tank water with 3 - 5 drops of Lugols' for 3 - 5 min max. I would cut the heads and dip right-away.
After that you leave them alone. Make sure there is enough water movement.

I don't see any pox on the third shot. I would wait to see what happens.

Give'm some water flow...

Check your water parameter to see if anything is out of limit. Did you add or changed anything laterly like light, flow direction or salt?
 
+1 with checking your light and flow. If you haven't changed anything physically like your lights or salt then there has to be something in your water chemistry that's causing this.
