radiant wrasse

hey everyone, i have had my radiant wrasse and solar wrasse for about 6 months now, and over the paast couple days i noticed the radiant kinda scratching. and he looks raw on both sides of his body behind the yellow head. i do not believe that ich is the culprit. been there done that. how ever i am wondering if it could be my substrate. i have argonite i believe. i am wondering if this is something that can be reversed or hopefully find out what is causing this. the rest of the fish are fine. they include 2 ocellaris clowns 3 chromis 2 engineer gobies and the solar wrasse. i did just add a new sump/fuge to the system as well as a new skimmer. tests are zeros for trite trate ammonia. phosphate is close to .25 and and all ideas and help are welcome. thanks for the read.

Is he scratching on the substrate or on the rock in your tank?

occasionally, but not constant. his skin is irritated for sure.

Could be intramuscular parasites or gill flukes. Some wrasses are prone to internal parasites.
