You can set it up however you like. I prefer adding a sand bed because it adds more sand to the system, but many are getting away from sandbeds altogether. Mine is a low flow area with sand and macros. The rock I emply in my fuge/sump arrangement is located in the sumop where the water falls from the tank (where the bioballs used to be/would be).
I just kept macro in mine when I ran one. If you do use sand(or even if you don't) push a bunch of flow through the fuge. If not lots of detritus will build up in there fairly quickly. I prefer no sand or rock so it can be cleaned out periodically. You will get plenty of critter growth in just chaeto.
