Save my Tang - Marine Ich or Marine Velvet?

I have attached some pics and need to know whether this is marine ich or velvet. I know he had marine ich before. Started treatment with Ich-X (not actualy something I'd recommend). He's really sick. He was seemingly getting better with no visible spots on him last night and in the morning he looks like this:

When was the last fish or something wet added to the tank? Source? (Ich-x does not work)

The tang is in a QT by himself. He has been out of my DT for the past 2ish weeks. I say Ich-X does not work partially from my own experience and also from customer reviews of the product.
I just want to be sure that this issue is still marine ich and not velvet. I know the black ich is another issue he has entirely.

he's also not swimming around any more. he's just swimming in position near the bottom of the tank .

I would begin a copper treatment asap, make sure the pH is above 7.9, helps the fish handle the toxicity of copper. I think it's severe ich, (definitely not velvet) it may not be treatable at this point, but always worth trying.
