Sick clownfish, help needed

I just got a juvenile GSM, and he was doing quite well the first couple days I had him, but I noticed last night and today that he was hiding a bit more than normal. I finally caught a glimpse of him, and he doesn't look well. It looks like there's some sort of infection around his mouth, and he's not eating. He's also not really swimming, just hovering around the bottom of the tank. His bottom fins aren't moving at all.

He's currently in my QT, and I'll double check the parameters, but before I put him in, Ammonia was 0, Nitrites were barely detectable at about 0.1-0.2, Nitrates weren't much higher at around 0.5-1.0. PH is currently 8.1, water temperature is pretty stable between 76-78 degrees, and salinity is at 35 ppt. Here's a picture. The white spots I'm talking about are just above the mouth, and it looks like they may be around the eye as well, but it's difficult to tell:
