Ok, so I checked Veron's books and I've got nothing on this thing. It came from about 90-110 ft in the Indo-Pacific if that helps, unfortunately it's dead now as you can see from the pics with the falling off flesh, but maybe someone can id...

how long did it take to die in your tank? i would think you would have to keep the tank temp at around 60 F with low light and spot feeding to keep it alive
 
It died from being imported unfortunately, wouldn't have needed feeding or colder water though, just lower light and slow acclimation if it would've lived :(

Actually I think it might be a new species, but in hindsight I probably should've titled this thread "new LE super rare unseen bad *** pocillopora".
 
It looks alive in the picture. Polyps are retracted. It died after the picture was taken?

Ya, looks alive to me. If it is still in the tank, I would suggest taking a couple frags off it. I would take one larger frag, and then several of the most perfect looking small pieces...even a couple teeny pieces with only a few polyps. Quite a unique piece...I would so all I could to try to preserve a piece of it!
 
I'm not a noob guys, the pics just don't show how bad of shape this one was in person, and unfortunately it vaporized in just a few hours. I think it came in with a sponge attached and just had no chance...
