As much as it bugs me when people automatically label something they cant ID as a 'hybrid', I have to say, this one really does look like a Ctenochaetus hybrid to me. It may be an odd looking C. flavicauda, but it looks an awful lot like a C. flavicauda x C. hawaiiensis. Its a little difficult to make out its face- is it a uniform color, or spotted?

Definitely Acanthurus genus, not Ctenochaetus genus. There are many Acanthurus genus that appear similar, and photodocumentation of juveniles is scarce... add to this regional variation and it could get confusing...

With that said, your fish appears to be Acanthurus grammoptilus (finelined surgeonfish), which ranges to Indonesia. Here is an adult photo from good old Jack Randall. Note the stripe in the caudal fin and lines through the body... Also note the lineation in the dorsal and anal fins... I need to check his "Reef and Shore Fishes of the South Pacific" book when I get home... another one of his great books that weighs more than my two year old...
