Tank problems - where to start?

OK, tough to write all this, I'll give a brief tank history leading into the problems I'm having. I'm heading out of town tomorrow and hoping there will be something left when I get back & trying to figure out a plan when I do get back. Thanks in advance.

Tank is about 4 years old.
180 gallon mixed reef w/ 75 gallon sump
AI SOL LED
Run carbon, GFO & Biopellets
25 gallon water change every weekend
Same fish have been in the tank since it was set up unless noted below.
SG: 1.026
Alk: 3.65
Calcium: 435
Mag: 1350
Phosphate: 0.04

Last fall everything looked as good as it ever had. Getting good SPS color & growth, LPS & softies looking good as well. Tank was around 3 years old at this point.

December: I added T-5 supplements to bring out more color and light the edges of the tank.

January: Still looking good. I fragged some of my SPS since it was growing so well.

March: The problems start. SPS pieces randomly started dying off. I would lose one piece, then it would hit another, then the next. Dying pieces would bleach at the base & die off within a few days. Overall bleaching of many other SPS pieces. But some pieces look & grow better than ever, some directly next to pieces that died off.

April: I cut off the T-5's and have not lost any other SPS as of today, but color of bleached pieces has not improved. Still, some look as good as they ever have.

No major changes until July.

July:
*Lost 2 of 3 heads on my gold torch. Other LPS looks fine. Not sure if a fish ate it or if it's related to the SPS problem.
*SPS basically same condition as of April. No further losses.
*Noticed a couple weeks ago my Kole Tang was not eating. He would graze algae but not eat prepared foods. Everyone else eating fine.
*Added a gold midas blenny two weeks ago. First fish added in years. He was harrassed by the current fish and died this weekend after I moved him to another tank. I did not QT. No obvious symptoms beyond being stressed.
*Monday - Kole tang died. Assumed it was due to pre-blenny issues.
*Yesterday - out of town for work & wife called saying 7 year old Royal Gramma died. I did not notice any symptoms prior to yesterday.
*Today - Flame angel on the verge of death. Lethargic w/ irritation around eyes & gills. Currently hiding under a pillar of rocks.

I don't know if everything is related or not. It's been about 6 months of things not looking very good from the SPS issues and now fish death. I did an extra water change and just fed the tank & everyone but the flame angel ate. So I hope I will get through the weekend but want some kind of plan for next week to start fixing the problem.

Sounds like some parameter is out of whack. Use whatever test kits (that are up to date) that you have and post your results. If you can get a hold of a Triton Water test, or get a hold of Triton US and ship them a water sample ASAP and get results for all your levels and see if that points you in a direction to start looking.
 
Thanks for the replies. I've been thinking about the Triton test so I think I'll take your advice and order a kit. Maybe that will point me in the right direction.

I initially suspected salinity was off as well, but I don't think that's it.
I just re-tested the salinity with two refractometers.
Milwaukee electronic: 1.026
Standard visual refractometer: 1.025
The Milwaukee read 1.028 w/ some older 1.0264 calibration solution so I may be a little lower but not enough to be causing all these problems.

Re-tested alk, calc, mag, PO4. Everything was the same or close to the original post. I used to test weekly and these are all within the normal tank variance.

I've kept up w/ my RO/DI system as well, last set of filters were changed out about a month ago.
