True percula looks bad! What is it?

I noticed a small white spot on my clowns mouth yesterday morning. Today I wake up and it looks bad, it also has a spot on its body and tail! Idk what it is? Plz help me diagnose and treat my fishy!! And idk if this has anything to do with it, but sometimes she leaves my GBTA, but she goes back after a while.

And on top of all this. I had got this fish from reefhotspot.com and they require a picture of the dead fish to get a replacment, but I forgot and I already flushed it!!! Uhhhh... this sucks. Would anyone happen to have a old picture they could send me?
