Unidentified Ballast

Need to identify a ballast that I sent off and now am having problems w/correctly identifieing who was the maker. The ballast was gold aluminum anodized with heat style vent on top much like a sound amplifier on the side it had a (I believe) female clip and on opposing side it had open end vented with a fan all I have is the address that was on the ballast it self when I received it I was told that it was for a dual VHO bulb system
