What all would make a fishe scratch?

I have two of my fish scatches on the sand every now and then There has been nothing new added to the tank in a month.Both fish are esting fine.I'm feeding mysis shrimp,emarald entree,marine cursine,seaweed delight,nori and formula 2.My tank is a FOWLR I have 1 cleaner shrimp.I have pimafix.Should i go ahead and dose my DT?There are signs of anything.I plan on starting soaking my food in vitachem and vitamen c (I cant' seem to spell today)next week.
 
I don't treat my fish if all I see is an occasional scratch/flash against a rock. Unless there are other signs of disease or you have reason to suspect a problem like flukes, I would feed well and watch them carefully for further symptoms. If scratching is the only symptom that you are seeing, you would only be guessing at treatment anyway.

Fish sometimes scratch against things from low pH, ammonia, virtually all external parasitic infections, and in my opinion sometimes simply because they are itchy.

If you have no corals and your only invert is the cleaner shrimp, you might do some reading about flukes and consider treating your tank with Prazipro (praziquantel), if you didn't do this during quarantine.
