what makes a coral or a clam to bleech

what is the cause of a clam or coral to get bleeched? what can you do? and how can you tell if it's bleeched or just white?

disease,bad water params, over or under lighting are some causes. a clam will completely waste away if it dies. only the shell will remain. a bleached coral is not a good sign. if it still has flesh u might be able to save it. if the flesh is gone its dead.
