What's wrong with my clam?

How to treat? What is it? Help!

Yes, and yes. From what I could tell. Was going to try and remove, but didn't want to harm it.
 
cant see much from the photo but to me it looks like a ball sponge but if you say its hard then i would say its a vermetid snail. both are harmless

Good news. Doesn't seem to bother him, so ill leave it be.

I would still take it out. It might impede the clam's full potential to open.

as long as he isn't sticking out his ugly face and tickling the clams mantle, he should be fine. the clam probably doesn't even know he's there.

if you can get a better pic or the situation changes, let us know
 
if you choose to remove him, it may require a little elbow grease to scrape the shell off but it shouldn't be that hard. the clam can come out of water, you just want to limit the time he is out or you may cause a spawning incident.
