What's wrong with my purple dottyback?

I've had this fish since last fall. About a month ago, I started noticing these kind of dull patches on it's side. I didn't make anything of it. They were on one side and weren't white, just a little more dull than the normal body color. It was a very agressive eater. Now, within the last week it's developed some weird problem with the lower portion of it's gill. I can't tell exactly what it is and it won't stay out long enough for me to get a picture. It's like a small white patch on the lower portion of the gill. It almost looks like the gill's torn and it's turned white but I can't tell. It's not breathing heavily. But it doesn't swim around much anymore and just hides in the dark most of the day, peaking out ocassionally. It won't eat.

It mainly would only eat frozen foods. I could never get it to eat flakes or pellets. So it's normal diet consists of frozen plankton, mysis, brine shrimp, squid, and would sometimes eat some prime reef flakes. I also use vibra-gro pellets, and formula 1 and 2 flakes. Can anyone give me a diagnosis? I've never seen something like this before, but it's the same thing my coral beauty was doing when I put it in the tank last week and now it's gone. Just would hide and not eat until it's now disappeared. I'm going to try doing a small water change and order some vitamin supplements.
