Why is my torch coral dying off

Hi, I've had this torch coral x 1 mo and for the last week, it's been slowly loosing heads. I thought it might be brown jelly disease so I dipped it in Coral Rx but it keeps progressing. Once it's dead, the peppermint shrimp eat the dead parts, never the live part of the coral. Tank has been established for 1 year. Feeding 3 times per week.

Parameters: Ph 8.2, temp 77, alk 12, Ca 450, mg 1360, nitrate 2, phosphate 0.08
 
The heads are getting torn to shreds trying to open up against the rocks and you're going to lose more if you don't find an open space for it, for future reference you need to give them more than enough space to open up instead of just a little bit around the skeleton.
