Yellow Tang with wound

I have a yellow tang that seems to be acting fine and still eating great. I've noticed in the past couple of days a wound on his lower side that looks like it's been rubbed so raw you can see his flesh.

I thought he might of touched my anemone and was burned or scared there. He doesn't pick on any fish and no fish picks on him. I can't tell if its just a wound or disease/bacteria.

Any ideas? I'll try and get a pic.
