Idk what causes this but some of mine do this occasionally. How long have you had it and has it opened up normally at all? I moved some of my polyp frags really high up in my tank they seem to be loving the place probably an inch under the water surface and I did my first water change a couple days ago since they have been there and one frag was out of the water for a little bit and they puffed up like that but returned to normal shortly after.

Yes this is totally possible. I had this same problem and was losing zoas to amphipods. My originally plan was to get a yellow coris wrasse to eat the pods. Want and to get one in beaver of weather. Kept delaying shipments, and then they died because of the delays. I ended up dosing Sentinel ... Yes the same thing they treat sps red bugs with. It kills small crustaceans too. It worked. Zoas doing fine now.
