2 Dead Fish?

So unfortunately I don't have a ton of info for you guys to help me with but I got a call from my wife today about my red scooter dragonet being dead, my green clown goby dead, and a missing clownfish. I've had all fish for 6 months to 2.5 yrs and haven't added anything to the tank other than corals in 4 months. I've had the clowns for 2.5 years. The tank is 6 months old and I test my water weekly.

There are no spots or marks on the fish. The only thing I noticed is the green clown goby was brown. I had noticed a few days before this that he had a dark color to him but I don't really see him much due to having Pukani rock and never comes out. The only thing I've done differently is add some algefix for some white hair algae from r/o filters needing replacement. I did so and got my phosphates back down. any ideas thoughts and opinions would help. Thanks in advance.
 
You say the fish are much older then the new tank? How did you change tanks? And did you add any new equipment and what equipment do you have and the tank size?
 
Yes I have a very very small yellow tang (about the size of a golf ball), one more clown (mated pair), and a green chromis. I also have a bubble anemone, many hermit crabs, coral banded shrimp, blue tuxedo urchin, and turbo snails that all seem to be ok. I haven't added any new equipment since I started the tank. The equipment is listed in my signature. It's the 40 breeder. I started this tank 6 months ago, cycled it, and added the scooter blenny. Then moved the clowns, yellow tang, chromis, clown goby, and coral shrimp from my 90 gallon setup that is 2.5 yrs old. All of these fish have been in the 40 for the duration since the tank was cycled. I added a flame angel from my 90 for a couple of months but it was picking at my sps so I got rid of him. The weird thing is I had a coral beauty in the tank a couple of weeks after it cycled with my clowns and suck. One night my wife came down and stated my coral beauty was dead. I checked it out for any spots, blemishes or defects and found nothing. All of the other fish mentioned were in the tank at the time and have been doing great until this point. Hopefully this helps.
 
Just noticed that my yellow tang has ich. I had moved a few corals the other day and my anemone from my 90 that had ich. I have all of my fish in qt now doing tank transfer. It's just weird that the dead fish had no spots and died so quickly.
 
So its a 125? And I don't see any powerheads listed do you have any?

The 125 is my new tank. The tank in question is my 40 breeder. I had moved a few things from my 90 to my 40 for the transfer (no fish) and I guess the anemone had sand on it's foot or something but now my 40 has ich as well. The 125 has 2 korilia evo's one 750 and one 1050.
