Anthia dies after using carbon?

As stated I have had 2 dispar anthias for a few months now. Both are dead within 2 wks after using carbon. Got the carbon from LFS and is good, not the recalled stuff thats out there. I had a large sinularia I had to peel off a rock and went well thats reason for using carbon due to its toxicity. All other fish are fine and tank params are good. Both anthias ate well and were active up until I found them dead. Could it be an internal parasite thing? The only thing I can think of that changed was the addition of carbon.
 
I think it's doubtful that non-recalled carbon (Kent) would lead to fish death. Especially since the other fish are doing fine. Parasites and flukes could be one of many possibilities. If you still have the bodies, I'd do a fresh water dip to see if any flukes fall off.
