Beheading Frogspawn

I purchased a single headed frogspawn about a year ago, it grew to having three heads and was getting quite massive. Then i moved three states away and during the move, it became damaged (it took two days and happened mid winter with temps around 0 overnight) and even though i kept it in a heated cooler, when the tank was reset, it started to shed the heads.

All three dropped but when the base receded, it left two polyps that started to grow. After about 6 months one died (overnight for no reason) and the other flourished. It was getting spot fed and showing significant growth. Then, two days ago, i came home and it popped off its base and was laying in the rock. I know this signifies the end of its life, does anyone know why a healthy frogspawn would just up and pop off its base? it had direct light and like i said, got spot fed usually every other day.

I can try and post photos later if someone thinks it would help.
 
An abrupt change or spike in things like temperature, salinity, alkalinity etc are known to cause the polyps to bail out like this.
