Blk Clown not swimming well and floating verticle?

I woke up this morning and noticed my black clown was laying upright in the sand as if he was resting. I gave food to see if he would eat and he does but he is not swimming fast and doesn't really move his back tail too much. He swims mostly with his fins on the side (pectoral I believe).

He is slow, floats verticle from time to time, breathing hard, not swimming normal, rarely using his back tail and he eats.

I hop he is ok and pulls through. He is my favorite fish and the one I've had the longest.
 
How long have you had it?
Have you added anything to the tank recently?
What size tank?
What all do you have in there -- livestock wise

I've had him for about 6 months.
65G reef tank
I have a orange f.p clown, LMB, blue spot goby, 5 chromis, 2 PJ cardinals, and a small baby blue hippo (i plan to move to a larger tank as he grows)
Parameters haven't tested everything but the important ones
PH-8.0
Trates: around 30
trites:0
Phosphate:.25
Ammonia I don't recall exactly bc i'm not home but wasn't bad

He's just acting weird and swimming verticle but still eats and will swim if he has to. Before I left this morning he was resting on top of the star polyp.
