Bloodworms on Chromis?

Hi all,

I came home today and one of my blue-green chromis was missing. I located him hiding in the rockwork.

He was breathing heavily and appeared to have what looked like bloodworms behind each of his pectoral fins.

I can't find any information on a condition like this. Is there anything I can do to save this fish?
