Branched hammer issue!

So it had this growth for sometime but it looked Happy and healthy so I ignored it. Fed it occasionally. Noticed last night it was very dark and shrunken. Then I see it's head is breaking off.

I have had mine for 5 months and it was doing great. Then within the last 3 days all 3 heads fell off.

Not sure on water parameters but i do 20 gallon water changes every two weeks.

Only thing that has changed in my tank was i got new Radions about 3 1/2 weeks ago.. maybe mine didn't like the acclimation period?

Could be your lights are too intense. Radions put out a lot of light. I had mine running at 60% and none of my corals were doing well. Hammer was shrunken all the time, Duncans were closed 90% of the time and my Frogspawn's polyps were barely open. Dialed my lights back to 40%, with a momentary peak of 45% at noon, and all of my corals look great now.
 
It's a five gallon nano. With one par38 with 90degree optics nine inches from the top of the water. All parameters look good but nitrates are a tad high. Today the hammer was fully opened. It looks like it is growing another branch off a head. So weird. This is my first tank. Everything else in tank is doing quite well. Should I try moving light up?
 
So both heads appear to be branching off.
