Bubble tip anemone suddenly died

Ok so I had a very healthy BTA and all of a sudden it died. It was trying to split and idk if it got sucked into the intake valve but either way, let me know what your ideas are. :-( (it died and literally disintegrated w/in three days)

What are your water parameters in numbers & how long has your tank been set up? There seem to be alot of nutrients in your water w/ that much algae in it.

This is a pretty dirty tank by anemone standards. The surface looks bad too. How much flow do you have in the tank? Can we get a full tank shot?
