Cardinal red spot? id please.

I just picked up a pair of cardinals and I don't know how the hell I missed this, but one of them has a red spot near its gill/pectoral fin.

It almost looks like an exposed gill, but is on both sides. Looking more closely, it looks like it might be cut on the pectoral fin, but I'm guessing the chances of that being the case on both sides slim to none.

Can you ID this disease/issue? and provide some assistance to treat?

I added prazipro to the 20L QT they're in.

The only other thing that can look like that is the impact of a pistol shrimp or mantis, which is not necessarily symmetrical. Heard any clicking from the tank?
 
btw that looks like ammonia poisoning to me.
That's what I first thought, but api test showed 0 ammonia. The next day after this photo, the red spots went away. Thought everything was going to be ok, but came home to him laying in the PVC pipe dead.

This was a new purchase so he was in a QT with another cardinal.

Unfortunately the other one is showing the same symptoms.
Red spot, and long stringy white feces.

Doesn't appear to be breathing heavily, and isn't swimming to the surface.

I got them both on Sunday, did a 50% WC on Monday night, first one dead on Tuesday night. Did another 50% WC hoping ammonia wouldn't spike due to the death and I'm not sure how long it was dead. Came home tonight and saw the other one with the red and stringy feces. Did another water change.

I just had a trio of anthias go through QT in this tank without issue. I moved them out and into the DT 4 days prior.
