Chrysurus Angelfish ID help

I see I never provided an update, sorry, but better late than never I suppose. I ended up not medicating for this condition, but rather just increased the volume of water changes. Although my test kits didn't indicate any significant water quality issue, obviously it was a factor as the condition improved almost immediately and never returned.

The fish is currently fat and healthy, and has been enjoying life in the DT for the last 6 months.

I was doing 50%wc every other day for 2 weeks and nothing changed
