clam is hurt...please help

my new clam has been doing fine, but today I added some tongan conchs, zebra hermits, and some smaller conchs.

and this afternoon I noticed the one side of the clam wasn't opening over the shell, so i took a close look, and it looks as if something has eaten a small little piece of it, it's a dersea, or however you spell it and it has a purple small small outline on the mantle, a little of the mantle is eaten away, so i'm wondering can he grow it back, or is it death?

and could bad water cause this? or is this more something eating it?

what else do you have in the tank?fish ,shrimp ,crabs,ect.why do you ask about bad water,what does everything test at?
 
everything tests fine, I added like 13 ml or cal and alk today, and then this afternoon, it was like this...

i have two cleaners, but they haven't touched him at all for a long time, and they have never even looked at him, they aren't on him now either...

i dont have any bad crabs or anything, the reason i say bad water, is because I always see a lot of stuff floating on the top of my water, like, lots of oils, I can stick my hand in the tank, and I have this weird film like dots on my skin...so now i have some filter running with carbon, it cleans it up fast...

do you think it will die? i will try to get some pics up soon...thanks

it hard to say. if it just a little nic then it should be ok but if something is picking at it then that's a different story. you don't have any fish?
 
2 ocellaris, 1 yellow tang, 1 blue tang, 1 royal gramma, 1 (brand new today goby, ray white stripe something)

and just the snails today, the conchs, they went under in the sand...should I put something over the clam to protect him until he heals?
